Help the Blacksmith

Gather the necessary materials and craft the horseshoe. Once approved by the blacksmith, follow the path you used for hauling sacks to the stables and deliver the horseshoe.

Help Fanka the Cook

Then, engage with the nearby woman, Fanka, who works as a cook in the noble's kitchen. Offering your assistance grants you access to the kitchen. Accompany Fanka to the kitchen; while not required, lockpicking it will boost your reputation and secure a standing invitation to fetch food whenever needed.

Speak with the Chamberlain

Screenshot by The Escapist Upon entering the kitchen, you'll notice the chamberlain in distress. Approach him and attempt to persuade him to let you prepare a Digestive Potion. Success grants you access to the surgeon's workshop; failure necessitates sneaking in.

Make Fever Tonicum

Otherwise, proceed directly to the workshop using the stairs opposite the forge, leading to the castle's non-noble kitchen. Navigate through the halls until you reach the surgeon's workshop, identifiable by the door on the right. Here, you can craft both the Fever Tonicum and, if you persuaded the chamberlain, the Digestive Potion, using the ingredients found in the room's chest.

Cure Thomas

If sneaking, you'll need to take an alternative route. Return to the noble's kitchen and ascend the stairs, avoiding guards along the way. As you approach a staircase with a guard at its base, distract him with a pebble to sneak past and reach Thomas' room. Hand over the Fever Tonicum to Adele, Thomas' sister, triggering the quest's concluding cutscene.
